PRE-ASSESMENT INTAKE FORM
(fill out 48 hours before initial coaching call)

INITIAL COACHING CALL
(40 minutes - goal setting, past dieting history, discuss struggles & pain points, food logs, mindset coaching)
Provide tasks to complete on check-in form before Month 1 Coaching Call

Monthly
2 x COACHING CALL (25 minutes)
- Lesson based on 1 habit based goal client wishes to work on
- Formulate plan for next several months together with measurable action items to create sustainable change in lifestyle
- Review check-in sheet and goals
- Delivering education lesson to help client continue to work on habit-based goal & achieve success

- Goals setting
- Anthropometrics + body composition
- Food logs
- Non-scale goals (energy, sleep, digestion, mood, etc.)
- Confidence checks
